Primary Health Solutions and Community First Solutions are expanding access to dental care for older adults through a new partnership focused on bringing services directly to residents. Beginning in April, on-site dental services will be provided at two Community First senior living communities, Berkeley Square and Westover, making preventive care more accessible in a setting that is comfortable and familiar.
Through this partnership, Primary Health Solutions dental hygienists will visit each community twice per month to provide professional cleanings and oral health assessments for assisted living and skilled nursing residents. Services are expected to expand to independent living residents in the future.
Community First Solutions is supporting the coordination of care within its communities by dedicating on-site multi-use space for visiting health professionals, helping ensure residents can easily access services in a trusted and familiar environment. This model reflects CFS’s commitment to integrating ancillary health services — including dental, vision, and behavioral health — as part of a comprehensive approach to resident wellness.
Following each visit, patients will also have access to a teledentistry (virtual dental) consultation with a licensed dentist. If additional treatment is needed, care teams from both organizations will coordinate follow-up visits at a nearby Primary Health Solutions location in Hamilton, ensuring continuity of care while minimizing barriers to access.
