Our board-certified Family Nurse Practitioners treat minor illnesses and injuries and can help you get back to feeling your best. If you’re already a PHS patient, our Express Care team will also coordinate follow-up care with your primary care provider.
We welcome all patients and all insurance plans. Sliding fee discounts are available based on financial eligibility.

After your visit, medications can be sent to any pharmacy you choose, including Primary Health Solutions Pharmacies in Hamilton or Middletown.
Express Care providers can:
- Provide acute medications for illnesses and injuries
- Offer medication refills (no controlled medications) for established PHS patients

Knowing where to go for care can save you time, money and stress. Whether it’s a routine visit, a sudden illness, or a serious emergency, choosing the right place helps you get the right care—right when you need it.
EMERGENCY ROOM Think: red lights and sirens. The ER is for life-threatening or very serious conditions that need immediate attention from doctors and specialists—available 24/7. If you’re experiencing chest pain, trouble breathing, severe burns, injuries or heavy bleeding, don’t wait—go straight to the ER.
DOCTOR’S OFFICE Where prevention happens. Your doctor knows your health history and can help you stay healthy, manage ongoing conditions and catch potential issues early. Visit your doctor for routine checkups, ongoing health concerns, medication refills or common illnesses like colds and minor infections.
URGENT CARE Care for the in-between. Urgent care (or express care) is faster and less expensive than the ER. It’s great for evenings, weekends or same-day needs. And, convenient for treating minor illnesses or injuries like sprains, ear infections, flu symptoms or small cuts needing stitches.
