By opting-in to this Program, you consent to receive Primary Health Solutions Communications Program messages via mobile text. Your consent to receive these text messages is not required as a condition of purchasing any goods or services.

Consent is required to participate in the Program, and you must be 18 or older to participate. Message frequency may vary.

Program communications may include messages about immunizations, clinical services, clinical trials, savings opportunities, or other health-related benefits and services that may be of interest to you. You agree that these messages may include limited personal information about you, and whoever has access to the mobile phone or carrier account will also be able to see this information. You acknowledge and understand that messages are sent without encryption, which presents some security risk. We take your privacy seriously.

To opt out of any individual campaign relating to the Program, please reply STOP to a text message. Or, to stop all messages associated with the Program, contact a PHS representative, such as a pharmacy or clinic associate. Opting out of Program messages does not opt you out of any PHS alerts unrelated to Program (for example, prescription or appointment alerts).
